Terms and Conditions for Booking

Heart Health Hub – Cardiac Diagnostic & Rehabilitation Services

Welcome to Heart Health Hub (“we,” “us,” or “our”). By booking an appointment with us for any cardiac diagnostic or rehabilitation service, you agree to the following terms and conditions. Please read them carefully.

1. Services Provided

Heart Health Hub offers a range of cardiac services including, but not limited to:

  • ECG (Electrocardiogram)
     

  • Holter monitoring
     

  • Stress testing
     

  • Echocardiography
     

  • Cardiac rehabilitation programmes
     

  • Lifestyle and risk factor assessment
     

  • Patient education and support services

 

  • Blood pressure monitoring

 

  • Blood pressure monitoring
     

All services are provided by qualified professionals and governed by UK healthcare regulations.

 

2. Booking Process

a. Appointment Confirmation

Your booking is not confirmed until you receive an official confirmation email or message from us. We reserve the right to reschedule your appointment due to clinician availability or unforeseen circumstances.

b. Accuracy of Information

You agree to provide accurate and complete information when booking, including your full name, date of birth, contact details, and relevant medical history. Providing false or misleading information may result in refusal of service.

 

3. Payment and Fees

a. Private Patients

For private bookings, payment must be made either in advance or at the time of service, unless otherwise agreed. We accept major debit/credit cards and bank transfers, 

b. NHS Referrals

If your appointment is via an NHS referral, no payment is required by you directly. NHS terms will apply, and cancellations may still affect your eligibility for rebooking.

c. Insurance Coverage

You are responsible for checking with your insurer that the service is covered. Any costs not covered by insurance must be paid by you.

 

4. Cancellations and Refunds

a. Patient-Initiated Cancellations

If you wish to cancel or reschedule, we require at least 48 hours’ notice. Cancellations made with less than 48 hours’ notice will incur a cancellation fee of up to 50% of the service cost.

b. Non-Attendance ("No-Show")

Failure to attend a booked appointment without notice will be treated as a no-show and will result in a full charge for the missed service. Repeat no-shows may result in service refusal.

c. Our Right to Cancel

We reserve the right to cancel or reschedule your appointment due to unforeseen circumstances, such as staff illness or equipment failure. In such cases, you will not be charged and we will do our best to reschedule promptly.

d. Refunds

All services are non-refundable, subject to review. 

 

5. Health and Safety

You agree to follow all health and safety instructions provided by our staff during your appointment. Failure to do so may result in termination of service.

If you are feeling unwell, have symptoms of an infectious illness, or have had recent exposure to a contagious disease (e.g., COVID-19 or measles), please contact us in advance to reschedule your appointment.

 

6. Confidentiality and Data Protection

We handle your personal and medical data in accordance with the UK GDPR and the Data Protection Act 2018. For more details, please refer to our Privacy Policy available on our website.

All patient information is treated as confidential and is only shared with third parties (e.g. your GP, specialist, insurer) with your consent or where legally required.

 

7. Clinical Responsibility and Limitations

While we strive to provide accurate diagnostics and appropriate rehabilitation services, no outcome is guaranteed. Diagnostic results should always be interpreted by a qualified clinician. Heart Health Hub is not liable for decisions made based on partial information or without follow-up medical care.

 

8. Code of Conduct

We reserve the right to refuse or discontinue services in cases of:

  • Abusive, threatening, or disrespectful behaviour toward staff
     

  • Repeated failure to attend appointments
     

  • Misuse of medical equipment or facilities
     

 

9. Changes to Terms

We may update these Terms and Conditions from time to time. The most current version will always be available on our website. Continued use of our services after changes implies your acceptance of the updated terms.
