Cardiac rehabilitation is a tailored programme that supports your physical recovery, emotional wellbeing and long-term heart health. It’s ideal if you’ve experienced a cardiac event, have a heart condition, recently had a pacemaker insertion or other cardiac surgery, or simply want structured guidance to improve your fitness and reduce future risk.

At Heart Health Hub, we are Northern Ireland’s only independent cardiovascular rehabilitation service — offering one-to-one rehab sessions delivered by highly experienced NHS-trained specialists. Our approach combines expert exercise guidance, lifestyle coaching, education and ongoing support, all centred around your personal goals.

What happens during your programme

Your rehabilitation begins with a full assessment of your medical history, current fitness level, symptoms, medications and lifestyle. From there, we create a personalised plan that may include:

  • Guided cardiovascular and strength-based exercise
  • Heart rate and blood pressure monitoring throughout
  • Breathing and mobility work
  • Symptom management strategies
  • Lifestyle and nutrition guidance/education
  • Stress and sleep support
  • Home-based or clinic-based exercise plans

Each session is delivered at your pace, with continuous monitoring to ensure you are progressing safely and confidently.

How to prepare

There’s very little you need to do in advance:

  • Wear comfortable clothing and supportive footwear.
  • Bring a list of medications if you take any regularly.
  • Bring inhalers or GTN spray if prescribed.
  • Eat and drink as lightly beforehand — you’ll be moving during the session.

If you prefer, rehab sessions can also be carried out at home.

What to expect

Your first session will include a relaxed conversation, a walking assessment and an introduction to your personalised programme. Every step is explained clearly, and exercises are adapted to your comfort level and ability.

As you progress, we’ll monitor your improvement, adjust your programme and help you build confidence in your daily activities. Many patients find rehab improves energy levels, mobility, mood, and overall quality of life.

Your results & ongoing support

We’ll track your progress across each session and provide clear updates on your improvements — such as stamina, strength, heart rate response and symptom changes. You’ll receive tailored advice on how to stay active, manage your condition and reduce long-term risk.

If additional tests such as an ECG, Holter monitor or echocardiogram would support your recovery, we can arrange these quickly — often within the same appointment block.
